This study investigates the impact of IBD on in-hospital outcomes in patients experiencing their first cerebrovascular event. Methods: A retrospective cohort study was conducted using the U.S. HCUP NIS Database (2016-2022). The study population included patients admitted for a first-time cerebrovascular event (acute ischemic stroke, transient ischemic attack, or acute hemorrhagic stroke) with a history of IBD (Crohn’s disease or ulcerative colitis), identified by ICD-10 codes. In-hospital mortality (primary), length of stay, resource utilization, and need for interventions (secondary) were analyzed. Multivariate analysis, adjusted for confounders, was performed. Results: Between 2016 and 2022, 694,399 patients were admitted for the first time with acute hemorrhagic stroke; 1,200 (0.17%) had concurrent ulcerative colitis. UC correlated with reduced in-hospital mortality, but increased length of stay. Among 3,691,869 patients admitted for acute ischemic stroke, 7,330 (0.20%) had UC and 9,615 (0.26%) had Crohn’s disease. UC was associated with increased length of stay and hospital charges in AIS patients. CD was associated with a lower likelihood of intracranial thrombectomy. Discussion: IBD patients experiencing a first cerebrovascular event exhibit different in-hospital outcomes compared to those without IBD. While in-hospital mortality may be reduced, length of stay and hospital charges are increased, suggesting a need for enhanced screening for cerebrovascular disease in this population.
