This study aimed to analyze the incidence of pancreatic cancer among patients with BD-IPMN. Methods: We identified 321consecutive patients with a diagnosis of BD-IPMN from 2008 through 2021. Age, sex, serum CA 19-9 level, location of BD-IPMN, number of IPMN, size of IPMN, diameter of main pancreatic duct were collected using medical records. Results: Demographic characteristics were as follows: mean age of 66.0 ± 11.3; 158 male (49.1%) and 162 female patients (50.3%). Of 321 BD-IPMN patients, 101 patients were followed-up over 5 years. The BD-IPMNs were located at the head in 49 (48.5%), body in 15 (14.9%), tail in 12 (11.9%), and multifocal in 25 patients (24.8%). Single neoplasm was observed in 68 patients (67.3%), 2-5 neoplasms in 28 (27.7%), ≥ 6 neoplasms in 5 (5.0%). Sizes were < 10mm in 8 patients (7.9%), 10-19 mm in 47 (46.5%), 20-29 mm in 35 (34.7%), and ≤ 30mm in 11 (10.9%). Median diameter of main pancreatic duct was 3.0 mm (range, 1.0-11.0). Pancreatic cancer developed in 8 patients (IPMN-derived, n=4; De novo, n=4). Five-year incidence and 10-year cumulative incidence of pancreatic cancer were HR 2.72 (CI: 0.182 – 5.252) and HR 10.270 (CI: 2.221 -18.320), respectively. Discussion: The cumulative incidence of pancreatic cancer among patients with BD-IPMN was gradually increased. Continuous surveillance for the development of pancreatic cancer is necessary.
