Hashash, MD, MSc, FACG2 1Mayo Clinic, Kansas City, MO; 2Mayo Clinic, Jacksonville, FL Introduction: Allergic contact dermatitis (ACD) represents a significant burden among the general population, yet its prevalence and risk factors in patients with inflammatory bowel disease (IBD) remain poorly understood. We aimed to assess the risk of ACD among patients with IBD. Methods: This retrospective cohort study analyzed data from 388,461 patients diagnosed with IBD, of which 4,845 were identified with ACD using the US-collaborative TriNetX database. Baseline characteristics, including disease type [Crohn’s disease (CD) or ulcerative colitis (UC)], sex, race/ethnicity, and common comorbidities and treatments, were described. The risk of ACD among patients with IBD was compared to a control group without history of IBD. Adjusted odds ratio (aOR) and 95% CI were calculated, adjusting for potential confounders such as age, sex, race, and ethnicity. Results: Among the 4,845 IBD patients with ACD, 44% had CD and 56% had UC. The cohort predominantly consisted of male (62.7%) and white patients (78.9%). Common comorbidities included asthma (24%) and rheumatoid arthritis (5%). The majority were treated with prednisone (75%) and a significant proportion received anti-TNF therapy (26%). Patients with IBD were at increased risk of ACD (aOR: 1.51; 95% CI: 1.45 – 1.57) compared to the control group. Additionally, both patients with UC (aOR: 1.45; 95% CI: 1.38 – 1.51) and CD (aOR: 1.39; 95% CI: 1.33 – 1.46) had a higher risk of ACD. Discussion: This study identifies a significant association between IBD and ACD. The findings may guide clinicians in the early identification and management of ACD in this population, enhancing patient care and outcomes.
