P0468 - Clinical Outcomes for Patients With Anemia and Early-Onset Colorectal Cancer: Further Analysis of the National Inpatient Sample

Introduction: Despite recent global decreases in overall cancer incidence, the incidence of early-onset colorectal cancer (EOCRC) is steadily increasing in western populations. Limited data is available on the comorbid correlations for this unfortunately expanding population. We sought to examine the national inpatient sample database to describe in-hospital outcomes among the anemic population with EOCRC.

Methods: Data were extracted from the National Inpatient Sample (NIS) Database for 2019 and 2020. The NIS was searched for hospitalizations of adult patients with EOCRC, defined as all-cause colorectal cancer in patients 50 years old or younger. We then examined the outcomes of patients who were identified as anemic. Multivariate logistic was used to adjust for confounders. The primary outcome was inpatient mortality. SPSS software was used for statistical analysis.

Results: This study included 11,820 patients with EOCRC, of which 4878 (41.3%) were found to have anemia. Anemic patients had a higher prevalence of hypertension (7.40% versus 3.90%, p< 0.001) and chronic kidney disease (9.67% versus 4.20%, p< 0.001). Multivariate regression showed that anemic patients with EOCRC had higher inpatient mortality (OR 1.775, CI 1.612-1.954, p< 0.001). On secondary analysis, patients with anemia and EOCRC were more likely to have metabolic dysfunction-associated steatotic liver disease (OR 1.643, CI 1.463-1.845, p< 0.001), thrombocytopenia (OR 2.560, CI 2.338-2.803, p< 0.001), acute renal failure (OR 2.445, CI 2.302-2.618, p< 0.001), all-cause arrhythmias (OR 1.913, CI 1.713-2.137, p< 0.001), and all-cause shock (OR 3.256, CI 2.508-4.229, p< 0.001).

Discussion: In this nationally representative, population‐based retrospective cohort study, anemic patients with EOCRC were associated with higher mortality and worse outcomes.
