Spanish-speaking patients often face language and literacy barriers that impair understanding of prep instructions. Studies have shown that this patient population has been particularly more difficult to achieve a higher quality preparation. This study assessed whether a culturally tailored Spanish-language educational video improves bowel preparation quality. Methods: We conducted a retrospective cohort study of 91 Spanish-speaking patients undergoing colonoscopy. Patients were divided into two groups: those who watched the educational video (n=39) and those who did not (n=52). The educational video was recorded utilizing the Doctorpedia platform(www.doctorpedia.com) and patients received the educational the morning prior to the day of colonoscopy. Prep quality was rated by the endoscopist as Good(BBPS 7-9), Fair(BBPS 4-6), or Poor(BBPS 1-3). We used a chi-square test to compare bowel prep quality across groups, with significance defined as p< 0.05. Results:
In the video group, 64.1% had Good prep, 28.2% Fair, and 7.7% Poor. In contrast, among those who did not watch the video, 34.6% had Good prep, 44.2% Fair, and 21.2% Poor. A chi-square test yielded χ² = 8.29 (df=2), with a p-value of 0.016, indicating a statistically significant association between video viewing and better prep quality. Discussion: Spanish-speaking patients who watched the video were nearly twice as likely to achieve good prep and three times less likely to have poor prep compared to those who did not. This supports the use of culturally and linguistically appropriate video education as a low-cost, scalable strategy to improve procedural outcomes and reduce health disparities. Broader implementation could help address prep-related barriers in limited-English-proficient populations. Future prospective studies should confirm these findings and explore integration with other support tools like text reminders or patient navigation.
