Methods: A hospital-based cross-sectional study. Participants were recruited from the oncology department of a tertiary care hospital in India between January and May 2024. Patients diagnosed with liver cancer and currently undergoing immunotherapy were included. Patient responses were collected using a structured, investigator-developed questionnaire that measured their knowledge, attitudes, practices, and lifestyle patterns in relation to irAEs and nutritional care during treatment. Results: A total of 402 patients participated in the study. The mean scores were as follows: Knowledge: 6.60 ± 3.51 (out of 10; 66.0%) Attitude: 41.26 ± 5.06 (out of 50; 82.52%) Practice: 30.74 ± 4.20 (out of 40; 76.85%) Lifestyle: 42.37 ± 6.04 (out of 55; 77.04%) Regression analysis revealed that higher attitude scores were significantly associated with improved practice behaviors (β = 0.381, p < 0.001) and healthier lifestyle habits (β = 1.928, p < 0.001). Patients living in urban areas were more likely to have higher attitude scores (β = 1.242, p = 0.013), as were those who lived with family or companions (β = 1.619, p = 0.044). Additionally, receiving more than one line of immunotherapy treatment was positively linked to better lifestyle scores (β = 1.928, p < 0.001). Female participants also had significantly higher lifestyle scores (β = 1.431, p = 0.023). Discussion: Patients with liver cancer receiving immunotherapy exhibited moderate levels of knowledge and favorable attitudes and practices concerning irAEs and nutritional support. Their lifestyle habits were also moderately healthy. Positive associations were observed between attitudes and both practices and lifestyle, while sociodemographic factors such as urban residence, cohabitation, female gender, and extended immunotherapy history further contributed to more favorable outcomes.
